fill in the blanks and return it to the address above.
Here is your 2012 annual membership renewal form. Please note that if you leave the club after October 1, 2011, you will have one year to spend any tournament winnings you have in your pro shop credit account. Any credit remaining in your account after one year will revert to the club treasury.
Your completed renewal form with your check made payable to Palo Alto Golf Club must be post marked no later than December 15, 2011. Please complete the form and return it in the enclosed envelope. Without exception, any renewal fee post marked after December 15, 2011, must include an additional $10.00 late fee.
Local Junior Golf activities continue to receive national attention. Adding $5 or $10 or more to your dues for Junior Golf will help immensely with this effort that is now supporting over 25 juniors in our club as well as in several local programs.

As you know the club maintains contact information in a secure on-line member directory at the club’s website. Access to this information requires your GHIN number and a password. If you have not yet used this feature, visit us at www.paloaltogolfclub.org. You can decline to be included in the on-line directory but that will also preclude you from using it. You can also be included but exclude certain information by indicating those fields below.
